Who qualifies for Direct Billing?
The availability of this service depends on your insurance provider and the specific plan you have.
When you come in for your initial consultation, please bring your insurance information, including your policy and ID number. Our friendly office staff will assist you in verifying your insurance coverage and determining whether your plan allows for direct billing. It’s important to note that the patient is responsible for all treatment fees.
If direct billing is not an option for your plan, you will need to pay for your visit upfront. We accept payment in various forms, including cash, debit (Interac), Mastercard, Visa, and American Express. To seek reimbursement from your insurance or extended health carrier, we will provide you with a receipt that you must submit to them.

Restore Life Physiotherapy & Wellness Waterloo is a private clinic, and therefore, we are not able to bill or charge OHIP or Ontario Health Card for our services. However, should you be receiving elective surgery for hip(s) or knee(s) replacements through an Ontario hospital, you may qualify for treatments and billing under the Bundle of Care Program by the Ontario Ministry of Health. Our clinic is registered with the hospitals, and we can bill with your Health Card for that program exclusively.
